I understand that if accepted as a volunteer, I agree to receive training to help me perform the volunteer activities. I affirm under the penalty of perjury that I have never been arrested or convicted of a felony offense. I certify that all information provided on this application is true and complete. I, also, understand that falsification or omission of any information may be just cause for non-acceptance or dismissal if discovered at a later date. In consideration of being allowed to volunteer at the Evansville Wartime Museum and in further consideration of the education and experience to be received, I, hereby agree not to bring or file claims or causes of action against the Evansville Wartime Museum or any persons acting as employees, agents, or officers as a result of any occurrence(s) during the period of time that I am working as a volunteer. The undersigned (volunteer) desires to volunteer their time and talents to perform a duty for the Evansville Wartime Museum without promise, expectations or receipt of compensation or reimbursement for services rendered. The volunteer has offered to perform services for the Evansville Wartime Museum for a number of hours to be determined by the volunteer and the volunteer leadership of the Evansville Wartime Museum, not less than one shift (minimum of four hours) or one event per month to remain active. The volunteer will advise the volunteer leadership if their schedule is such that they will be unavailable for an extended period of time. Following an inactive period, volunteer agrees to attend a refresher shift or event
